Voluntary Dependent Term Life and AD&D

Eligible employees may purchase additional coverage for Term Life and AD&D for eligible dependents. Spouse coverage may be purchased in increments of $5,000 up to 50% of the employee benefit. Child coverage purchased in increments of $2,000 up to a maximum of $10,000 for Life; up to $20,000 coverage for AD&D.

Eligibility: New Hires become eligible on the first of the month after 90 calendar days of employment. Enrollment changes subject to Qualifying Events and Open Enrollment Periods.
