Group Health Benefits: Medical, Dental & Vision Care

Medical
Capital Insurance Group provides a comprehensive Medical Program at extremely competitive rates for all full-time employees and eligible dependents. Our Medical Program offers a PPO Plan (Preferred Provider Organization). The Plan covers In-Network charges at 80% and Non-Network charges at 60%. The Annual Calendar Deductible is $200 per individual and $500 per family.

Dental
Capital Insurance Group Dental coverage is available for full-time employees and eligible dependents. The Plan provides for 100% preventative care; 80% basic services - class 2; and 50% major services - class 3. Deductibles apply to class 2 & 3 services. The Annual Calendar Deductible is $25 per individual and $50 per family.

Vision
Capital Insurance Group Vision Program covers full-time employees and eligible dependents. There are no deductibles. The Plan offers a schedule of benefits for exams and lenses every twelve months and frames every 24 months.

Eligibility: New Hires become eligible on the first of the month after 90 calendar days of employment. Enrollment changes subject to Qualifying Events and Open Enrollment Periods.
